ATTENTION ALL APPLICANTS:

All applicants will be required to provide the Board with a unique email address. In our current system, several email addresses which are listed for individual licensees are actually business email addresses or group practice email addresses. Those with such addresses should contact the Board immediately at (702)732-2147 to provide us with an email address that is unique, not shared, dedicated and distinct to each individual licensee. Your immediate attention to this matter will be greatly appreciated!

New DO and PA Applications Accepted Online

To obtain an application for licensure to practice medicine as an osteopathic physician or physician assistant in the state of Nevada, you may utilize the Board's online application process, by clicking the link below, or by calling the Board office at (702) 732-2147.

You should make no commitments for employment in Nevada until you have completed the licensure process and received your Nevada medical license number.

New AA Application Accepted By Paper Only

To obtain an application for licensure to assist in the practice of medicine as an anesthesiologist assistant in the state of Nevada, you may utilize the Board's paper application process, by clicking the link below.

Please note that the addendum on the last page of the application should not be submitted to the Board until after the regulations have been published in the Nevada Administrative Code and you and your supervising osteopathic physician have read them.

You should make no commitments for employment in Nevada until you have completed the licensure process and received your Nevada medical license number.

Oral Exams or Interview are NOT required of Every Applicant

However, the Executive Director or the Board may request your appearance at a Board meeting to consider your license if they have any questions regarding your application.

Fingerprint Card for Criminal Background Check Required for Full License Applicants

All full license applicants (those NOT requesting special licenses) shall submit a fingerprint card for a criminal background check.

Jurisprudence Exam

We no longer require the Jurisprudence Exam, however if you click on the "Laws and Regulations" tab, this will lead you to our Nevada Revised Statutes and Regulations; it is strongly recommended that you read through these and familiarize yourself with the laws that govern your profession. Any questions you may have regarding Osteopathic Medicine in the State of Nevada are in the NRS and NAC and just by reviewing these you will find that 99% of your questions will be answered. Once again, this is strongly recommended!

Federation Credentials Verification Service (FCVS) Report

The Board requires the Federation Credentials Verification Service (FCVS) report (or a primary source verification) on ALL applicants for full licensure. You may enroll in the FCVS by visiting their website at www.fsmb.org. Please allow a minimum of 6-8 weeks for the FCVS to verify your core credentials.

Once Application is Complete

Once your application is totally complete, your application will be scheduled for the next available Board meeting. You may check the status of your application in the online portal. 

Nevada Pharmacy and Nevada Controlled Substances Certificates

You will need to apply separately for your Nevada Pharmacy and Nevada Controlled Substances certificates AFTER you receive favorable action on your application. You may get more information at Nevada State Board of Pharmacy web site.
